The Total Actual Cash Expenses listed above must equal or exceed the amount of the Grant Amount Requested above.  Note: Expenses may exceed $500, with the additional income provided by the artist or organization as listed as "Income."  However, the applicant is not required to contribute matching funds.  While cash matches are not required be sure to inclue personal contributions, organizational funds (if applicable) and any other outside funding. 

IV. Supporting Documents

Upload one letter of recommendation from someone with professional qualifications in the arts, who knows the quality of your, or the organization's work, and who can act as a knowledgeable reference.

Upload resumes or bios for the applicant and any other key personnel to be involved in the proposed project. If attending a conference or educational opportunity, please upload a brochure or catalog or copy thereof, describing the class.

Upload a detailed budget of all cash expenses and income (collated into the application). This breakout should provide a description of how funds will be spent. While cash matches are not required, be sure to include personal contributions, organizational funds (if applicable), and any other outside funding.

Upload Promotional materials, if available, from past activities. Include newspaper articles, concert programs, etc.

Upload submit work samples – one (1) copy if possible that are directly related to this proposal such as: slides, audio or video recordings, literary writing, musical scores, etc. After the granting process, samples of work can be picked up from our office.

V. Certification

Any grant funds received in conjunction with this application will be expended for activities as described above.  Any changes to this application or budget must be submitted in writing and are subject to the approval of the Salina Arts and Humanities Foundation.

Applicant hereby agrees to comply with Title IV of the Civil Rights Act of 1964, Section 504 of the Rehabilitation Act of 1973, and where applicable, Title IX of the Education Amendments of 1972.  Title IV, Section 504, and Title IX prohibit discrimination on the basis of race, color, national origin, handicap or sex.

Grantee’s financial records relating to this proposal must be maintained for a period of three (3) years and are subject to periodic audits by SAH.  In addition, SAH reserves the right to monitor the grantee to ensure that applicable terms of the grant are being met.
