In consideration for authorization to volunteer my services in connection with the performance of the activities described above ("Activities"), I, the undersigned, for myself, my heirs, personal representatives and executors, and with full knowledge of all risks involved, agree to assume all risks of participating in the Activities and to fully and forever release and hold harmless the City of Salina, Kansas, and all of its agents, representatives, officers, officials, and employees (collectively, the "City"), from any claims, causes of action, or demands, of any nature or cause, based on any injury, death, damage, loss, or expense suffered by me as a result of or in any way connected with my performance of the Activities. I understand I am a volunteer for all purposes, including workers' compensation, and I will not be considered an employee, agent, or representative of the City. By serving as a volunteer, I will not be entitled to employee benefits of any kind. The City of Salina has the authority at any time and in its sole discretion to terminate my participation as a volunteer, with or without cause. I acknowledge and agree that, in performing the Activities, I will not operate any powered equipment, except only a lawn mower, weed-eater and/or blower that I may own and voluntarily use in connection with the Activities. Any equipment that I utilize to perform the Activities shall be in good working order at the time of its use. I agree to report any personal injuries or damages to facilities or equipment arising from the Activities, as soon as possible to the Salina Parks and Recreation Department, 300 W. Ash St. Room #100 Salina, KS 785-309-5765. By signing, I confirm that I am 18 years of age or older and that I have read and accept these terms and conditions.
